Human Services Home Vital Records (Birth & Death Certificates)Sarasota County Health Department provides certified copies of birth
and death certificates to eligible individuals. Stop by one of our
locations or mail an application to the Ringling address. A legal
photo ID is required, along with the application. The cost for a
birth certificate is $15.00 for the first certificate and $5.00 for each
additional certificate purchased at the same time for same individual.
The cost for each death certificate is $10.00.
